Heart failure services

Our nurse-led Heart Failure service offers inpatients support from a multi-disciplinary team in the management of Left Ventricular Systolic Function (LVSD) EF<45%.

We use echocardiogram, symptom control, medicines, education and a discharge plan with referral to community heart failure nurse.

Outpatient clinics are provided for more complex patients; for example those awaiting revascularisation or device therapy.

We also support the end of life pathway, both in hospital and by liaising with community palliative care teams. We provide clinical resources to all community heart failure nursing teams and support data entry to the National Heart Failure Database.
